What Helps Us Assess Quickly

Please have the following to hand for the call. Do not email clinical documents to us from an ordinary mailbox — when we call you back we will give you our secure NHS.net address, and NHS.net to NHS.net is the route we use for all of it:

  • Current nursing assessment or care plan
  • Decision Support Tool (DST) if CHC funded
  • Recent medical history and discharge summary
  • MUST assessment or Waterlow score
  • Behavioural risk assessment (if applicable)
  • Equipment requirements list
